The Physician will serve as a Program
Physician for AppleGate---s clinic in Greenville, TX.----Physician shall
exercise independent professional judgment in the conduct of any activities
that may reasonably be considered to constitute the practice of medicine while
performing professional services to AppleGate---s clinic in Greenville, TX.----Physician
shall provide patient care, medical evaluation, establish dosing levels, and
interact as necessary with federal, state, county and other regulatory
bodies.----Physician may train and manage subordinate physician
extenders and provide health care services directly to patients all under the
administrative direction of the Administrator of AppleGate---s clinic in
Greenville, TX.----For all activities that constitute the practice of
medicine the Physician shall work with AppleGate---s National Program Physician
or Physician Consultant.--Essential Roles and ResponsibilitiesServe as a Program Physician for
AppleGate---s clinic in Greenville, TX.Interviewing, managing, training, and
evaluating physician extendersImplementation and enforcement of
medical policies, procedures and trainingAssistance as needed with development
of clinical policies, procedures and trainingAssistance as needed with completion of
Federal, state, county and other regulatory applications, permits, licensure,
inspections, etc.Correspondence and interaction with key
regulatory (DEA, SMA, Medicaid, SAMHSA/CSAT, etc.) personnel and accrediting
bodies as necessary.Providing reports & other data as
requested to AppleGate---s National Program Physician or Physician Consultant.Providing leadership to patients---
treatment teams including counseling staff, nurses, and medical assistants.--Patient CarePlanning and supervising provision of
treatment for patients (including regular review and notes in the patient---s
record) according to established CSAT and state guidelines with a strong
emphasis on quality and customer service.Signs patient record notesOversees patient care provided by
physician extender, signs charts, dosing levels, etc., when applicablePlaces patients into treatmentOversight of any patient site
emergenciesResponds to after-hours calls as neededExamines patient, performs
comprehensive physical examination, documents health history and examination
results to meet federal and state guidelines and refers patient for care
elsewhere when indicated (including appropriate follow up)Initiates, alters and terminates
replacement narcotic therapy medications following strict protocols meeting the
approval of federal and state guidelines as well as current medical literature
for patient careSupervises the administration and
dispensing of medicationsConfers with patient---s private
physician as neededExamines staff after on-the-job
accidents or injuries as neededServes as a clinic in-service resourceProvides patient education specific to
various disease processes within our scope of practiceOversees the continuum of care process
for patientsMaintains strict patient, employee,
company and computer confidentialityAdministrative ResponsibilitiesParticipation in federal and state
opiate/substance abuse trainings, education programs, etc.Inputs clinical findings in the patient
charts and electronic medical recordsPerforms (and documents) chart reviews
of all subordinate physician extendersInterviews, trains, educates, evaluates
and counsels subordinate physician extendersParticipates in meetings,
teleconferences, etc. as neededParticipates in AppleGate Performance
Improvement Process and accreditation processContinually reviews and enhances
medical policies, procedures and protocolsCompliance with HIPAA policy and
procedures







































































Compliance with Federal, State, and
Local regulations, in addition to the standards required by CARF, if
applicable, Board of Health, and OSHA, CSAT and the State Methadone Authority
